[CROI 2026] MedPage Today: Age, HIV, and metabolic syndrome together tied to cognitive impairment risk

Clinicians can target metabolic syndrome as the risk triad’s treatable factor.

The triple combination of age, HIV, and metabolic syndrome seemed to have a particularly potent impact on cognition, according to an observational study of more than 1,500 men.

Of the three factors alone, only age was significantly associated with a worse Global Deficit Score. But among men 65 years or older, those with both HIV and metabolic syndrome experienced significantly greater cognitive impairment than those with neither of the conditions.

The findings point clinicians to treatable targets that may help protect patients’ cognition. While the risk factors of age and HIV can’t be changed, “you can modify metabolic syndrome — it’s an actionable factor that can impact cognition.” Especially for older adults with HIV, that means optimizing blood pressure, glycemic control, lipids, weight, and other lifestyle factors that can contribute to worse cognition.
