Mpox among people living with HIV

A rapidly emerging multicountry outbreak of human mpox virus was first detected in Europe in May 2022, circulating primarily among sexually active men who have sex with men (MSM). Cases spread internationally, leading the World Health Organization (WHO) to declare it a Public Health Emergency of International Concern (PHEIC) on July 23, 2022. As of September 19, 2023, the WHO had recorded 90,439 cases across 115 countries, with 157 deaths. People living with HIV (PLWH) have been affected disproportionately, accounting for between 36% and 70% of cases, and experiencing more severe symptoms and higher rates of mortality.

An article, published in Infectious Disease Special Edition, reviews the evidence of the relationship between HIV and mpox, and explain why the disseminated form of mpox that occurs in people with advanced HIV-related immunosuppression should be designated an AIDS-defining disease. Recent signs of a resurgence in mpox cases, including a cluster of cases in Chicago in April and May 2023, underscore the continued need for advances in understanding mpox disease and the challenges faced by those with advanced HIV-related immunosuppression, many of whom may be either undiagnosed or disengaged from HIV care.
