Major real-world study affirms: PrEP is good — and the more PrEP, the better

There is a significant, inverse, dose-response relationship between usage of pre-exposure prophylaxis (PrEP) and HIV diagnosis rates, according to a large, long-term, state-by-state analysis published in The Lancet HIV. The findings are our clearest real-world evidence to date that PrEP works to curb HIV at the population level—and that we still have very far to go in making use of its potential, particularly for people of color.

Highlighted Study Population and Methods

This study assessed PrEP use, HIV viral suppression, and trends in HIV diagnoses in all U.S. states and the District of Columbia using a nationwide commercial pharmacy database (IQVIA).

PrEP coverage (measured as PrEP users per 100 people with a PrEP indication) and HIV diagnoses (based on public health surveillance data) were calculated by state for each year from 2012 through 2022. States were grouped into quintiles by their degree of change in HIV diagnosis rates.

After controlling for state-specific viral suppression, the authors calculated state- and national-level associations of PrEP coverage with HIV diagnosis rates.
