TheBodyPro analysis

The reasons men who have sex with men discontinue PrEP in the US are as varied as the reasons they start it in the first place, suggests research published in the journal AIDS. While many stop due to changing personal circumstances, others face individual and systemic barriers to continuing PrEP.

As a result, experts say, re-engaging people with PrEP requires individualized, multipronged strategies that include affirming whether reinitiating PrEP is actually appropriate for a particular person’s situation.

Key takeaways

  • Over 3,000 men who have sex with men completing the American Men’s Internet Survey explained why they had stopped taking PrEP.
  • Three profiles emerged: changed circumstances (35%), mostly altered sexual activity; structural barriers (24%); and clinical/multi-barrier (41%), frequently involving side effects.
  • Privacy and discrimination concerns doubled between 2018 and 2024, and were most often experienced by young men.
