It’s been a year since FDA gave the green light to lenacapavir for PrEP, our first six-monthly injection for HIV prevention. Late-stage clinical trials touted near-perfect efficacy — but how have things actually worked out in the clinic?

New results from a US study on implementation outcomes remind us that trial data only gets us so far — and also gives us some important context to take into the next conversation we have with a patient about starting PrEP.

This analysis reports the results of a retrospective multi-center cohort study of lenacapavir (LEN) PrEP from 17 academic and community-based HIV prevention programs in the United States. The study evaluated individuals who received at least one LEN injection between June and December 2025 and abstracted data from standardized electronic health records.
