Modern antiretroviral therapy (ART) regimens have reshaped clinical outcomes and quality of life for people living with HIV. But for individuals who are highly treatment-experienced, resistance and tolerability issues may limit treatment options.
A significant proportion of people who have taken several HIV treatment regimens exhibit resistance to at least one ART agent, with some studies identifying two-class resistance in nearly 60% of participants and three-class resistance in up to 6% of participants. In addition to transmitted or acquired drug resistance, inconsistent access and suboptimal adherence to ART constitute significant challenges that increase the risk of virologic failure for individuals with a long history of treatment.
Long-acting ART options have equipped providers with new tools for addressing the unmet needs and preferences of a broad spectrum of patients. Novel ART classes with improved tolerability and no cross-resistance to other agents may help optimize regimens for individuals with resistance to multiple drug classes.
